Bottle Sipper Caps Recall Registration
Hazard: The pull-up valve can come off, posing a choking hazard to young children.

Incidents/Injuries: Bradshaw International has received one report of a pull-up valve coming off. The valve came off while a child was drinking from the bottle sipper cap, and started to choke.

Description: Each retail package contains three plastic bottle sipper caps on a blue backing card. The front of the blue card is labeled “Bottle Sippers…Fits Most Water Bottles.” The back of the card has the item #70118 and the Bradshaw International name. The screw-on section of the cap is white and each package has one cap with a blue, green and red pull-up valve.

Sold at: The bottle sipper caps were sold at a variety of retail outlets from January 2002 till August 2005, for approximately $2.00 per package.
